SOCIAL SECURITY
We will notify Social Security that there has been a death in your family immediately after the arrangements have been made.
Note: We will only notify them -- WE DO NOT MAKE A CLAIM!
To
apply for any benefits, you must call the Social Security office. You
may call 1 (800) 772-1213 to file a claim or call the Montrose office
(249-1072) located at 30 N. Uncompahgre, Montrose, CO.
AVAILABLE BENEFITS:
- A one-time benefit of $255.00
If...there is a surviving spouse or dependant Child.
- Disabled widow's benefit (Age 50-60)
Proof of disabililty must be provided.
- Mother of children under age 16
- Dependent children
- Disabled children
- Divorced but married for 10 years or more
If it is necessary to go to the Social Security office, remember that they work on a first come, first served basis.
LIST OF INFORMATION THAT SOCIAL SECURITY MAY ASK YOU TO PROVIDE:
- Death Certificate
- Social Security numbers (Both yours and that of your spouse)
- Date and place of marriage or marriages
Take your marriage certificate if possible)
- Military - dates of entry and discharge after 1939.
Also, branch of service.
- Date and place of birth of deceased
- Work record for past 12 months
- If surviving children benefits apply:
- Dates of birth of children
- Social Security Numbers of children
- Birth certificates of children

VETERANS BENEFITS:
Taylor Funeral Service and Crematory & Mesa View Cemetery will file for the applicable burial allowance:
- Veteran died in V.A. facilites; or
- Veteran died enroute after being accepted by V.A.;or
- Cause of death was a service connected disability.
Taylor
Funeral Service will also secure a flag and headstone upon request. A
copy of the form DD 214 (separation notice) is required to secure these
