SOCIAL SECURITY

We will notify Social Security that there has been a death in your family immediately after the arrangements have been made.
Note: We will only notify them -- WE DO NOT MAKE A CLAIM!

To apply for any benefits, you must call the Social Security office. You may call 1 (800) 772-1213 to file a claim or call the Montrose office (249-1072) located at 30 N. Uncompahgre, Montrose, CO.


AVAILABLE BENEFITS:


  1. A one-time benefit of $255.00
    If...there is a surviving spouse or dependant Child.


  2. Disabled widow's benefit (Age 50-60)
    Proof of disabililty must be provided.


  3. Mother of children under age 16


  4. Dependent children


  5. Disabled children


  6. Divorced but married for 10 years or more


If it is necessary to go to the Social Security office, remember that they work on a first come, first served basis.


LIST OF INFORMATION THAT SOCIAL SECURITY MAY ASK YOU TO PROVIDE:


  1. Death Certificate


  2. Social Security numbers (Both yours and that of your spouse)


  3. Date and place of marriage or marriages
    Take your marriage certificate if possible)


  4. Military - dates of entry and discharge after 1939.
    Also, branch of service.


  5. Date and place of birth of deceased


  6. Work record for past 12 months


  7. If surviving children benefits apply:

    • Dates of birth of children


    • Social Security Numbers of children


    • Birth certificates of children

VETERANS BENEFITS:

Taylor Funeral Service will file for the applicable burial allowance:


  1. Veteran died in V.A. facilites; or


  2. Veteran died enroute after being accepted by V.A.;or


  3. Cause of death was a service connected disability.


Taylor Funeral Service will also secure a flag and headstone upon request. A copy of the form DD 214 (separation notice) is required to secure these items. V.A. INSURANCE:

Contact Veterans officer in the Courthouse for G.I. Insurance or Taylor Funeral Service will assist in filing for V.A. Insurance.
